Health Care Solution: Leave Request

• The purpose of the Vacation Request Form module is to show the logged in Physician a history screen Physician Leave Application Verification List indicating leave taken by him/her. The status field indicates whether the leave has been approved or not.
  

• The module also provides the physician ability to request for leave through the Physician Leave Application Form.
